Sade Akande-Jones
Sade Akande-Jones is a Registered Occupational Therapist in
good standing with the College of Occupational Therapists and a member of the
Ontario Society of Occupational Therapists. Ms. Akande-Jones has seven
years of experience working as an Occupational Therapist including extensive experience
with adults with neurological impairments in both inpatient and community
settings.
She is experienced in neurological assessment and treatment
(including cognitive, perceptual, upper extremity and psycho-emotional aspects
to assessment), functional assessment/intervention in the home and community,
and comprehensive psycho-social assessment.
Ms. Akande-Jones is currently practicing as an Occupational
Therapist with Complex Injury Rehab Inc. and provides services to clients in
Toronto and surrounding areas. Her current practice includes occupational
therapy in home assessment (including assessment of attendant care needs, home
safety assessments, and comprehensive physical, cognitive and psychosocial
assessments), discharge planning assessments and occupational therapy treatment
for clients with a brain injury, spinal cord injury, other neurological
conditions, chronic pain and orthopedic injuries. She also has experience
in equipment prescription, splinting and hand therapy.

Shannon Orr
Shannon
Orr is a Registered Occupational Therapist in good standing with the
College of Occupational Therapists of Ontario (COTO), and a member of
the Ontario Society of Occupational Therapists (OSOT). She graduated
from University of Toronto with a Masters of Science in Occupational
Therapy (M. Sc. OT). In addition, she holds a Bachelor of Science in
Therapeutic Recreation from Dalhousie University.
She is experienced in comprehensive functional evaluations of
individuals who are experiencing difficulties performing their essential
tasks of daily living due to illness and/or injury. She also has
extensive experience completing In Home Occupational Therapy assessments
focusing on functional, cognitive, psychosocial and emotional aspects
of injury. Ms Orr also has a good working knowledge of Job Demands
Analysis, Functional Abilities Evaluations and Ergonomic Assessments.
Areas of practice include orthopedic injuries, head injury, chronic
pain, diabetes, arthritis, Parkinson’s disease and return to work. Ms.
Orr is currently practicing as an Occupational Therapist with Complex
Injury Rehab Inc. and provides services to clients in York Region, South
Simcoe, Durham and Peel Regions.

Miranda Mo
Miranda
Mo is a qualified Occupational Therapist in practice within the
province of Ontario since 1991. She is registered and in good standing
with the College of Occupational Therapists of Ontario. Professional
affiliations include being a member of the Ontario Society of
Occupational Therapists. She is also qualified as an ADP authorizer, a
certified ARCON FAE evaluator, a certified PGAP clinician and has
recently completed a 44-hours course in utilizing the AMA guides (4th
edition) to the evaluation of permanent impairment for Catastrophic
Determination according to the SABS. In addition, she is in the process
of becoming a Canadian Certified Life Care Planner through the
University of Florida.
Ms. Mo’s professional experience focuses on delivering occupational
therapy services in community practice. Her practice is geared towards
community based assessments/treatments. Assessment experience includes
assessment of attendant care needs, cognitive assessments, work site
assessments, physical demands analyses, ergonomic assessments,
functional capacity evaluations and in-home functional assessments.
Treatment experience includes assistance to clients following hospital
discharges and coordination of their care in the community, designing
and implementing return to work and return to activities of daily living
programs and providing comprehensive Occupational Therapy treatment
services. Ms. Mo has over twenty years experience in evaluating the
physical, functional and psychosocial abilities of injured individuals
and in providing occupational therapy treatments to restore functions
for clients who are limited in the performance of their Activities of
Daily Living as a result of musculoskeletal, neurological, cognitive or
psychosocial impairments.

Jennifer Wraight
Jennifer
Wraight is an Occupational Therapist in practice within the province of
Ontario since 2004. She is registered and in good standing with the
College of Occupational Therapists of Ontario. Professional affiliations
include being a member of the Ontario Society of Occupational
Therapists.
Her career, including employment experiences and continuing
education, focuses primarily on the pediatric population. Her practice
includes assessment, consultation, and rehabilitation of children with a
variety of developmental, musculoskeletal, neurological, cognitive
and/or psychosocial impairments. Her experience includes assessing and
addressing functional fine motor and early gross motor issues, sensory
processing and integration difficulties, feeding and swallowing
disorders, and consultation in the areas of writing aids, seating and
mobility, home renovation/ modifications, and classroom participation.
She has over six years experience in evaluating the physical, functional
and psychosocial abilities of children and in providing occupational
therapy treatments to promote and/or restore functions for clients who
are limited in the performance of their Activities of Daily Living.

Nisanne Tarek
Nisanne Tarek is a Registered Occupational Therapist in good
standing with the College of Occupational Therapists and a member of the
Ontario Society of Occupational Therapists. Ms. Tarek has seven years of
experience working as an Occupational Therapist including extensive experience
with adults and older adults with neurological impairments in inpatient,
outpatient, and community settings. She is a certified NDT therapist.
She is experienced in neurological assessment and treatment
(including cognitive, perceptual, upper extremity and psycho-emotional aspects
to assessment, and is NDT certified), functional assessment/intervention in the
home and community, and comprehensive psycho-social assessment.
Ms. Tarek is currently practicing as an Occupational Therapist
with Complex Injury Rehab Inc. and provides services to clients in Toronto and
surrounding areas. Her current practice includes occupational therapy in
home assessment (including assessment of attendant care needs, home safety
assessments, and comprehensive physical, cognitive and psychosocial
assessments), discharge planning assessments and occupational therapy treatment
for clients with a brain injury, spinal cord injury, other neurological
conditions, chronic pain and orthopedic injuries. She also has experience
in seating and mobility devices, including prescription of power wheelchairs
and scooters.

Rehab Support Workers |
Jeremy Davies
Jeremy Davies has a Bachelor’s Degree from University College London, UK, and a post-graduate certificate in Education. He has more than 17 years experience working with children and adults with disabilities. For the past three and a half years, he has focused on rehabilitation with an ‘all things are possible’ attitude.
Jeremy has been a Rehabilitation Support Worker (RSW) with Complex Injury Rehab. Inc. for over three years and provides services throughout the GTA and beyond. His current practice encompasses home based and community rehabilitation support to address OT, SLP and PT treatment goals and includes: implementation of pool and gym fitness programs, cognitive remediation programs, community/college based educational support, community reintegration, education of Client’s and caregivers and a broad range of sporting / recreational activities.
Jeremy is experienced at supporting individuals with mild to severe physical and cognitive disabilities including those with brain injuries, spinal cord injuries, mental health disorders, soft tissue and orthopedic injuries, chronic pain and amputations. He is 100% Client focused and counts transparency and communication as interpersonal strengths.

Casey George
Casey George is a certified Kinesiologist and a Rehabilitation Therapist with 10 years of experience providing physiotherapy, occupational therapy, speech language pathology and psychology directed support to clients with acquired brain injuries, spinal cord injuries and complex orthopedic injuries. He is a member in good standing with the Ontario Kinesiology Association and regularly attends academic and professional development courses to ensure best practice and innovative service delivery. He has completed Level 1 and Level 2 training courses in Acquired Brain Injury through the Ontario Brain Injury Association.
Mr. George is currently practicing as a community-based Rehabilitation Therapist with Complex Injury Rehab Inc. and provides therapy and support services in Barrie and the surrounding area. He has an extensive knowledge of multidisciplinary rehabilitation practices and collaborates effectively with his fellow professionals to maximize client outcomes. As a Certified Kinesiologist Mr. George also works with clients to implement their home and/or community based exercise programs, including pool therapy programs. He is also a certified member of the Reflexology Association of Canada and can integrate reflexology treatment into his practice as appropriate.
