Life Care Planning/ Future Care Costs Analysis

A Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized concise plan for current and future needs with associated costs, for individuals who have experienced catastrophic injury or have chronic health care needs. [1]

The purpose of this report is to outline the recommendations of a person's treating professionals, make any additional recommendations that will enable him/her to live as normal a life as possible, and to ensure that he/she has adequate and reliable assistance to maximize his/her functional recovery.

In preparation of this report, items or services that will allow the person to maintain and promote his/her independence, reduce medical complications, maximize his/her function, and maintain a reasonable quality of life are allocated.

Our Life Care Plans or Future Care Costs Analysis are written by Occupational Therapists.
